Permissions
Permission to reproduce College publications must be sought well before the expected publication date of your document. You must submit a written request containing the following information:

1. Precise citation of the document or selection you wish to reprint
2. Title and publisher of your publication
3. Means by which your publication will be distributed (e.g., included in a book, a patient education packet, handouts to students, etc.)

Permissions are typically granted to reproduce selections of current College materials based on the following conditions:

1. Full credit to the source is shown as indicated in the permission granting letter.
2. The information must be reproduced without modification, with the exception of style and format changes.
3. Permission is given for one-time use and does not extend to electronic rights, subsequent editions, or foreign translations.
4. Permission is contingent on payment of a fee of $100.00 per selection.

Permissions are not granted to reproduce College material in any of the following circumstances:

1. Use of all or most of a publication (e.g., an entire Patient Education Pamphlet, a chapter from a book, or a form)
2. Electronic reproduction of material in any form
3. Use of material that is not original with the College (the original publisher must be contacted)
4. Reproduction of a document that has been withdrawn from circulation

Permission is not required for use in a descriptive article written about a College publication (e.g., book review) or for any other use permitted by law.

Photocopying

Permission is not necessary to make up to 5 photocopies of the College’s document for one's personal use.
Requests for authorization to photocopy larger quantities should be directed to the Copyright Clearance Center, which is authorized to grant rights to distribute the College’s material in exchange for a fee for noncommercial purposes. Reporting Copyright Infringements

The Digital Millennium Copyright Act, signed into law on October 28, 1998, amended the copyright law to provide limitations for service provider liability relating to material online. Subsection 512(c) of the copyright law provides limitations on service provider liability with respect to information residing, at direction of a user, on a system or network that the service provider controls or operates, if the service provider has designated an agent for notification of claimed infringement by providing contact information to the Copyright Office and through the service provider's publicly accessible website. 17 U.S.C. 512(c).
The College has designated an agent to receive notification of alleged copyright infringement occurring in any domain owned by the College.  Proper notification must include the following elements: 

• A physical or electronic signature of a person authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
• Identification of the copyrighted work claimed to have been infringed, or, if multiple copyrighted works at a single online site are covered by a single notification, a representative list of such works at that site.
• Identification of the material that is claimed to be infringing or to be the subject of infringing activity and that is to be removed or access to which is to be disabled, and information, reasonably sufficient to permit the College to locate the material.
• Information reasonably sufficient to permit the College to contact you, such as an address, telephone number, and, if available, an electronic mail address at which you may be contacted.
• A statement that you have a good faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
• A statement that the information in the notification is accurate, and under penalty of perjury, that you are authorized to act on behalf of the owner of an exclusive right that is allegedly infringed.
